Community-Based Charitable Contribution
Request Form
To request a Community-Based Charitable Contribution from MSD, use the Medtronic
Foundation U.S. Grant Application Form available at http://www.medtronic.com/foundation/apply.html.
Except in unusual and urgent circumstances, anyone requesting a Community-Based
Charitable Contribution of $2,500 or more within the Memphis metropolitan area
must complete the appropriate request form and provide all requested documentation
and information.
For smaller contribution requests, you can, but are not required to use the
designated form. Otherwise, send a written request with relevant information
to the person identified below.
In order to assure readability, MSD prefers that all request forms be typed
rather than handwritten.
Submit the completed and signed form and all of the requested documents and information to:

Types of Acceptable Community-Based Charitable Contributions for the Memphis Metropolitan Area
The types of Community-Based Charitable Contribution request MSD may approve are those requests that evidence a purpose to:
- Support MSD's mission.
- Build partnerships and empower people with chronic diseases, preferably diseases of the spine.
- Support education in the community, particularly science or health-related educational programs.
- Contribute to the vitality and improvement of the community.
- Improve the quality of life in the community
- Educate employees about community issues and MSD's involvement in the community.
Generally, Community-Based Charitable Contributions are not provided for fundraising events, even if the fundraising event is intended to support one of the above-stated purposes.

Types of Unacceptable Community-Based Charitable Contributions for the Memphis Metropolitan Area
MSD will generally deny contribution requests for:
- Capital and capital projects
- Fiscal agents
- Fundraising events/activities, social events or goodwill advertising
- General operating support
- United Way programs (United Way is supported through annual employee drives)
- Individuals, including scholarships for individuals
- Lobbying, political, or fraternal activities
- Long-term counseling or personal development
- Program endowments
- Religious groups for religious purposes

Tax-Exempt Charity Requirement
Generally, Community-Based Charitable Contributions will be provided only to tax-exempt
charitable organizations, including K-12 educational institutions.
